- Number of words used.
- The ISBN, title, page number(s), and author of the
work you desire.
- Details on how the material will be used.
- How will the material be distributed, in what style
and format (book/magazine), and how many copies will be
distributed?
- What is your geographic area of distribution?
- Who are the intended/anticipated users of the
material?
- Will the material be given away or sold? If sold,
what will be the price?
- Contact information on the person or the company
making the request: Full names, address, phone number,
fax number, email address, and website.
- One paragraph about the person or the organization
making the request, length of time in business.
- The names of other US publishers you have worked
with in the past.
- Copy of the material when printed with page(s)
marked.
